Use this planning worksheet to gather the information you'll need before beginning the online Community Assessment.

COMMUNITY INFORMATION
Describe the size and general population of area: is the population less than 50,000?

Estimate the number of people with a disability who can’t drive who live or work in the area.


Determine the availability, affordability and accessibility of any buses, taxi cabs, or dial-a-ride vans for persons with disabilities.


Determine the transportation needs and resources of area hospitals, nursing homes, or adult service providers.


List names and contact information for transportation funding sources operating in your state.


Determine the types of transportation programs these sources already provide in the area.


Survey the unmet needs among the disability community: is lack of transportation a significant issue for people in your area who don't drive?


Assess the capacity of current providers to expand to meet unmet needs.


YOUR AGENCY INFORMATION
Describe not-for-profit or government status  of your agency.


Describe  the  location of voucher site:  is it adequate for the workload? Is it accessible to persons with disabilities?


Indicate the availability of a computer with Internet access and a printer at your voucher site.


Estimate the number of staff or volunteers who will complete four hours of online training and work one hour per week on voucher management.


Estimate the  amount of start up funding that is available: do you have at least $30 per rider?


Estimate the number of riders who need vehicles with lifts, ramps, tie-downs or special equipment.


Describe the type and availability of special transportation vehicles and equipment.


DRIVER INFORMATION
Indicate the number of people in the community who will serve as volunteer or paid drivers.

  
Describe driver willingness to inform his or her insurance company about volunteer driving.


Describe the coverage or certification for drivers offered by area insurance companies.


Describe volunteer driver willingness to keep records such as beginning and ending odometer readings and submit data for payment.
