Partnerships Program

The Partnerships Program offers case aide and care coordinators for children with severe emotional disorders  and other intensive in-home family preservation services for children ages 21 and under.

What are severe emotional disorders?  These may include and are not limited to:

  1. Oppositional Defiant Disorder
  2. Bi-Polar Disorder
  3. Schizophrenia
  4. Attention Deficit Disorder with Hyperactivity
  5. Anxiety Disorders
  6. Conduct Disorders
The Partnerships Program has these goals:
  • Build parent/professional partnerships to better support families
  • Connect families to all the services they may need
  • Reduce out of home placements whenever possible
  • Help families coordinate all the care for smooth, non-confusing service delivery
  • Provide intensive services that match family need
To get support from the Partnerships Program, contact the Human Service Center nearest to you to see if your family is eligible for this unique service. Ask for the care coordinator of the Partnerships Program.
